Our practice collects information from you for the primary purpose of providing quality health care. We request that you provide us with your personal details and medical history so that we can properly assess, diagnose, treat and be proactive in your healthcare needs. The information you provide is confidential and will only be used as below:
- Administrative, in order to run our clinic
- Billing, including compliance with Medicare and Health Insurance Commission requirements
- Disclosure to others involved in your health care, including doctors, optometrists, allied health workers and specialists outside this practice. This might occur through referral to other practitioners or for medical tests and in the reports or results returned to us following the referrals.
In addition to our professional and ethical obligations, Inner South Eye Surgeons handles your personal information in accordance with federal and state privacy law. This includes complying with the Australian Privacy Law Act which was amended on the 12th of March, 2014, as well as the federal Australia Privacy Principles, which form part of the Privacy Act 1988 (Cth) and the Victorian Health Privacy Principles, which form part of the Health Records Act 2001 (Vic).
Your health information will be treated with the utmost confidentiality. Disclosure will not be made to any person not involved in either your treatment or the administration of this practice, without your written consent.
